Consultation and co-ordination of services
Professional consultation with licensed director, and co-ordination of all steps throughout the cremation process, including services and supplies provided by us, as well as those provided by third party suppliers, such as death notices.

Using an unmarked service van. Additional mileage fees apply beyond 80 km.

Documentation
Completion and filing of all documents necessary to carry out the services and supplies requested, including Municipal Death Registration, Coroner's Cremation Certificate. Provision of 10 proof of Death Certificates and AfterCare Paperwork - issued by Forest City Cremation Services.

Our Services Include Transportation from the Place of Death, Filing Required Documentation, Unlimited Number of Proof of Death Certificates (same day issued), Basic Mortuary Cremation Container (casket alternative), Transportation to the Crematorium, Service Vehicle, Coroner’s Fee and City Hall Registration Fee, Completion and Filing of the Canada Pension Death Benefits for Lump Sum (if eligible $2500.00) as well as filing for the Spousal Benefit.
The Cremation Fee is determined by the crematorium selected.
Financial Assistance is available to those that are eligible. We are in full cooperation with both Ontario Disability Support Program (ODSP) and the Discretionary Benefits Program through the City of London.
